KOKH-TV signed on the air on February 2, 1959 as an educational independent station founded by Independent School District No. 89 of Oklahoma County (now Oklahoma City Public Schools), which acquired the dormant KTVQ license.

On October 1, 1979, KOKH-TV converted into a commercial independent station (the first such station in the state of Oklahoma), adopting a general entertainment format consisting of a mix of cartoons, classic sitcoms, religious programs, some sports programming, and certain network programs preempted by NBC affiliate KTVY (now KFOR-TV), ABC affiliate KOCO-TV and CBS affiliate KWTV.

KOKH was sold to John Blair and Co. in the spring of 1979, becoming the company's second broadcast television property. On August 1 of that year, the station converted to a general entertainment independent format.

On August 15, 1991, KOKH-TV converted into an affiliate of the Fox network, taking over the affiliation from KAUT-TV (UHF channel 43). The switch resulted from Heritage Media's donation of the KAUT license and assets to the Oklahoma Educational Television Authority (OETA).
The OETA had planned to develop a secondary PBS member station for the Oklahoma City market since 1988, when it was involved in an aborted plan in which Pappas Telecasting Companies attempted to acquire KOKH with the intent of consolidating the entertainment programming inventories of KOCB (channel 34) and KAUT — including the Fox affiliation — and migrating them onto KOKH's programming schedule. Whereas the Pappas proposal intended to make KOKH the market's lone "independent" station, the Heritage deal only eliminated KAUT as an independent competitor.

This variant of the 2006 logo, which resembles the "kitebox" logo designs first used by Fox's owned-and-operated stations, was originally used on-air in primarily select news promotions and for weather branding purposes when it was introduced in November 2017. This variant became KOKH's primary logo in March 2018, when it was extended to the logo bugs used on the station's newscasts and syndicated programs.
