Wanted sa Radyo premiered in 1994 on DZXL.

Coinciding with Raffy Tulfo moving to Radyo5, Wanted sa Radyo changed its logo. along with the launch of the short-lived TV Edition on TV5, Wanted sa TV months later.

On January 13, 2019, during Aksyon TV's rebrand to 5Plus, Wanted sa Radyo was moved to One PH on Cignal Channel 6 (now Cignal Channel 1). This title card was updated on November 27, 2018 and it is still seen on One PH broadcasts.
From October 18, 2021 to October 20, 2021, Wanted sa Radyo was temporarily renamed to RTIA Online Live and went off-air on Radyo5 and One PH as a part of the program's migration to it's new studios from TV5 Media Center.
On October 21, 2021, with the program continuing to air new episodes on Radyo5 and One PH, the program was renamed back to Wanted sa Radyo with RTIA Online Live being an hourly online-only extended edition of the program, thus splitting the program into 2 parts.
On May 10, 2022, Tulfo returned to the show after the 2022 Elections, reverting its title card to normal.
2022[]
From February 8, 2022 to May 9, 2022, Wanted sa Radyo temporarily removed Raffy Tulfo's image on its title card, due to the said host running for senator in the 2022 Election Campaign. This title card was short-lived.
2023–2024[]
On March 9, 2023, due to the rebranding of the radio station to 92.3 Radyo5 True FM, the old logo of Radyo5 was replaced by a new one. However, the title card is still the same. This title card can still be seen on One PH, RPTV, and True TV broadcasts.
On February 1, 2024, Wanted sa Radyo started its simulcast on RPTV, marking its return to analog free TV, while it started its simulcast on its parent channel, TV5, on February 21 until April 5, 2024.
2024–present[]
Coinciding with the transfer of the station's frequency to 105.9 MHz and the launch of True Network, the True FM logo was changed and the addition of the True TV logo.

Despite Wanted sa Radyo vastly changed it's logo in 2012, Wanted sa TV still uses the 2011 logo design with minor changes in it's logo design.
Wanted sa TV ended on TV5 in July 30, 2012. but continued airing it's Radio Edition on Aksyon TV.
Wanted sa TV would later be replaced by another extension of Wanted sa Radyo, Wanted: Ang Serye. a docu-drama anthology premiered on January 16, 2021 on TV5 which was then replaced by Sine Spotlight in Late August/Early September 2021. A third extension, Idol in Action, was premiered on June 8, 2020 which was then replaced by the new timeslot of Frontline sa Umaga on October 4, 2021.
